Important Notice – New Policy
We have always been honest and transparent with our community and those we support, so to continue this we wish to advise we have introduced a new policy around the hiring of our equipment to protect Friends of Palliative Care Inc. after recent damage and mistreatment, which has been rather costly to the organisation to repair and re purchase equipment.
We wish we didn’t need to put this into place, however after multiple incidents over the past month it has left us with no other option. Effective immediately:
For any EQUIPMENT requests:
- Please visit our website to see the full list of equipment available
- Please download & complete the Loan Request Acknowledgment which requires 3 signatures
- Please photocopy/photograph the form and leave the original with the patient/NOK
- Once the form is completed, please upload it whilst also completing ALL fields on the web request form
- We will not be able to action any requests, until the form is completed in full and signatures have been obtained.
For any PHARMACY ACCOUNT requests:
- Please complete the forms attached
- Please photocopy/photograph the form and leave the original with the patient/NOK
- Email completed form to friendsofpalliativecare@gmail.com
- We will not be able to action any requests, until the form is completed in full and signatures have been obtained.
